Specialized Services- Service Code 116 PT

Our specialized services offer physical therapy to clients aged 0-3 through the regional center (service code 116), in a family-centered environment via center-based, in-home and telehealth services. Our therapists will teach the caregivers how to integrate interventions into their daily routine. The sessions and home programs will not only address routine activities of daily life, but also skills that will improve the quality of life for the child and the family as a unit. We will target functional goals that are meaningful to the child and their family in the context of everyday living in their natural environment.

Our Clinical Director of ABA Services

Jazmine Chavez, is a graduate of Purdue University with a Master’s degree in Special Education. She is Board-Certified Behavior Analyst (BCBA) and has extensive clinical experience in psychology, education and childhood development. With close to 15 years of professional experience, Jazmine’s expertise spans across Southern California, now currently statewide, including consulting for high-profile families. Her dedication extends to involvement in research and proposal writing for community programs dedicated to those with special needs. Jazmine’s approach centers on identifying the origins of behavioral issues, addressing gaps, and nurturing holistic growth and development. She has worked extensively in various settings, from private practice to school districts, in-home, and remote settings, serving individuals from two years old to adults with a diverse range of developmental and physical disabilities. She is passionate about empowering individuals with special needs to lead independent, fulfilling lives. When she is not working, Jazmine enjoys spending time outdoors.
